Mapleton Local School District follows Ohio Department of Health's required immunization schedule for school attendance. Immunizations are required before entering school, before entering 7th grade, and before entering 12th grade. Your students immunizations must be up-to-date at the start of each school year. If students are found to be out of compliance, the school holds the right to exclude the student after 14 days into the school year per Ohio law. Missed school days for immunization non-compliance are counted as unexcused absences.
In Ohio, a parent/guardian has the right to request an exemption from immunizations based on medical, philosophical, or religious reasoning. The Exemption Form is available here and must be completed EACH school year.
Flu Vaccinations are not required however we do offer them onsite in the fall through a partnership with Health Heroes. Dates will be announced and the required consent form is available here.
